Consent for Cognitive testing * - This document is a consent form to allow a Baseline ImPACT (Immediate Post-concussion Assessment and Cognitive Testing) and a Post Concussion ImPACT, as necessary. This information is embedded in the registration process. For reference only.

Medical Form A * - This is the information provided by parents. Please complete this form and return it to the Moorestown High School Health Office.
Medical Form B * - The medical evaluation is to be completed and submitted to the Moorestown High School Health Office. Rowers must have a physical that is dated within the last 365 of the first day of practice.

Novice Rower Meeting Flier - Information regarding the meeting at the high school for novice rowers and their parents.
Release of Liability - This is the Cooper Boat House release required once a year for each rower. Each rower will be advised when it is required. Print, sign, and submit to the coach.
Sponsorship Information - Information regarding the fund raising program through sponsors. Tax deductible donations!
Sponsorship Letter - This letter may be sent to businesses or individuals to assist with the fund raising campaign for Moorestown Crew. Tax Deductible!
